Job summary

The Care Team Hospice in Highland Heights, OH is seeking a compassionate Hospice RN Case Manager to coordinate patient care, develop plans of care, and work with physicians and families to ensure comfort and dignity at end-of-life. This full-time role follows a Monday–Friday schedule with on-call opportunities for additional compensation.

Qualifications

• Active RN license required.
• Hospice nursing experience preferred.
• Experience with Homecare Homebase (HCHB) preferred.
• Strong assessment, communication, and care coordination skills.

Responsibilities

• Provide skilled nursing care, assessments, and symptom management per hospice standards and plans of care.
• Complete scheduled visits, admissions, and clinical interventions with timely documentation in Homecare Homebase (HCHB).
• Respond promptly to patient, family, and caregiver needs, including after-hours on-call situations.
• Collaborate with physicians, caregivers, and interdisciplinary team to coordinate care and support outcomes.
• Educate patients and families on symptom management, disease progression, medications, and end-of-life care.

Schedule & Work-Life Balance

• Monday–Friday, 8:00 AM – 4:30 PM
• On-call opportunities available with additional compensation
• Some locations offer dedicated on-call teams, resulting in backup coverage only
