Job summary

The Care Team Hospice in Akron is seeking a compassionate Hospice RN Case Manager to join our dedicated care team. You will coordinate patient care, develop individualized plans of care, and work with physicians, families, and the interdisciplinary team to ensure comfort and dignity.

Active RN license is required; hospice experience is preferred. Schedule is Monday–Friday, 8:00 AM–4:30 PM with on-call opportunities and additional compensation in select locations.

Qualifications

• Active RN license required.
• Hospice nursing experience preferred.
• Experience with Homecare Homebase (HCHB) preferred.
• Strong assessment, communication, and care coordination skills.

Responsibilities

• Provide skilled nursing care, assessments, and symptom management per hospice standards and physician orders.
• Complete scheduled visits, admissions, and clinical interventions with accurate documentation in Homecare Homebase (HCHB).
• Respond promptly to patient, family, and caregiver needs, including after-hours and on-call situations.
• Collaborate with physicians, caregivers, and interdisciplinary team to coordinate care and support positive outcomes.
• Educate patients and families on symptom management, disease progression, medications, and end-of-life care.
