Job summary

The Care Team Hospice seeks a compassionate RN Case Manager to coordinate patient care, manage plans of care, and collaborate with physicians and the interdisciplinary team to ensure comfort, dignity, and quality of life for patients at home.

Schedule is Monday–Friday, 8:00 AM–4:30 PM with on-call opportunities and additional compensation. This role includes a comprehensive benefits package and opportunities for growth within a supportive, mission‑driven environment.

Qualifications

• Active RN license is required.
• Hospice nursing experience preferred.
• Experience with Homecare Homebase (HCHB) preferred.
• Strong assessment, communication, and care coordination skills.

Responsibilities

• Provide skilled nursing care, assessments, and symptom management per hospice standards.
• Complete scheduled visits and documentation in Homecare Homebase (HCHB).
• Respond to patient, family, and caregiver needs, including after-hours and on-call situations.
• Collaborate with physicians, caregivers, and interdisciplinary team to coordinate care.
• Educate patients and families on symptom management and end-of-life care.

Key Responsibilities

• Complete scheduled visits, admissions, and clinical interventions while maintaining accurate and timely documentation in Homecare Homebase (HCHB).
• Respond promptly to patient, family, and caregiver needs, including after-hours and on-call situations, to ensure exceptional patient-centered care.
• Collaborate with physicians, caregivers, and interdisciplinary team members to coordinate care and support positive patient outcomes.
• Educate patients, families, and caregivers on symptom management, disease progression, medications, and end-of-life care while providing compassionate support.
