Job summary

The Care Team Hospice is seeking a compassionate RN Case Manager to join our team in the Highland Heights/Willoughby area. You will coordinate patient care, manage individualized plans of care, and collaborate with physicians, caregivers, and the interdisciplinary team to ensure comfort, dignity, and quality of life for patients at end of life.

Schedule is Monday–Friday, 8:00 AM–4:30 PM with on-call opportunities and additional compensation.

Qualifications

• Active RN License required.
• Hospice nursing experience preferred.
• Experience with Homecare Homebase (HCHB) preferred.
• Strong assessment, communication, and care coordination skills.

Responsibilities

• Provide skilled nursing care, assessments, and symptom management per hospice standards.
• Complete admissions, visits, and clinical interventions with accurate documentation in HCHB.
• Respond to patient, family, and caregiver needs, including after-hours on-call.
• Collaborate with physicians, caregivers, and interdisciplinary team to coordinate care.
• Educate patients and families on symptom management and end-of-life care.

Key Responsibilities

• Provide skilled nursing care, assessments, and symptom management in accordance with hospice standards, physician orders, and individualized plans of care.
• Complete scheduled visits, admissions, and clinical interventions while maintaining accurate and timely documentation in Homecare Homebase (HCHB).
• Respond promptly to patient, family, and caregiver needs, including after-hours and on-call situations, to ensure exceptional patient-centered care.
• Collaborate with physicians, caregivers, and interdisciplinary team members to coordinate care and support positive patient outcomes.
• Educate patients, families, and caregivers on symptom management, disease progression, medications, and end-of-life care while providing compassionate support.
