Job summary

The Care Team Hospice in Ohio is seeking a compassionate Hospice RN Case Manager to coordinate patient care, develop individualized plans, and support families at end of life.

You will collaborate with physicians, caregivers, and the interdisciplinary team to ensure comfort, dignity, and quality of life for patients wherever they call home.

The role offers Monday–Friday, 8:00 AM–4:30 PM with on-call opportunities and a comprehensive benefits package to attract and retain compassionate caregivers.

Qualifications

• Active RN License required.
• Hospice nursing experience preferred.
• Experience with Homecare Homebase (HCHB) preferred.
• Strong assessment, communication, and care coordination skills.

Responsibilities

• Provide skilled nursing care, assessments, and symptom management per hospice standards.
• Complete visits, admissions, and interventions with timely Homecare Homebase documentation.
• Respond to patient/family needs, including after-hours on-call, for quality care.
• Collaborate with physicians and the interdisciplinary team to coordinate care.
• Educate patients and families on symptom management and end-of-life options.

Key Responsibilities

• Provide skilled nursing care, assessments, and symptom management in accordance with hospice standards, physician orders, and individualized plans of care.
• Complete scheduled visits, admissions, and clinical interventions while maintaining accurate and timely documentation in Homecare Homebase (HCHB).
• Respond promptly to patient, family, and caregiver needs, including after-hours and on-call situations, to ensure exceptional patient-centered care.
• Collaborate with physicians, caregivers, and interdisciplinary team members to coordinate care and support positive patient outcomes.
• Educate patients, families, and caregivers on symptom management, disease progression, medications, and end-of-life care while providing compassionate support.
