Qualifications

• Graduate from an accredited School of Nursing; ADN required; BSN preferred.
• Current license to practice as a Registered Professional Nurse in New York State.
• BLS required, plus specialized certifications as needed.

Responsibilities

• Assess the patient’s physiologic health status and document findings.
• Assess patient’s psychosocial health status and plan appropriate care.
• Identify patient outcomes and develop individualized goals.
• Formulate and coordinate the plan of care with the health team.
• Implement the plan of care and ensure safety, accuracy, and timeliness of interventions.
• Evaluate care effectiveness and update care plans as needed.
• Collaborate with physicians and care team; provide direction to care staff.
• Provide skin care and wound management as part of decubitus care.
• Administer medications correctly and educate patients and families on drug interactions.
• Provide patient/family education and support for discharge planning.
