ComForCare is seeking a Agency Supervisor/Director of Nursing Services. The DON oversees clinical compliance with NC/SC regulations and accrediting agencies, including care management and personnel management related to clinical delivery. The role also involves caregiver orientation, ongoing evaluation, and ensuring clinical skills and competency of caregivers (including skilled nurses, aides, and therapists).

Essential Duties and Responsibilities:
  1. Partner with the respective ComForCare business owner on clinical matters.
  2. Engage with clients through care planning and excellent customer service.
  3. Provide clinical oversight and case management as required by regulations or business line.
  4. Coordinate client care and manage transitions across the continuum.
  5. Facilitate caregiver orientation, competency, and skills validation.
  6. Ensure caregiver skills match client requirements across settings.
  7. Supervise specific client, caregiver, or staff populations as directed by regulations.
  8. Collaborate with the office team on documentation review.
  9. Educate team members, patients, families, caregivers, and referral sources.
  10. Manage caregivers and staff on assignment, assess concerns, and provide coaching or disciplinary actions as needed.
  11. Participate in clinical quality initiatives.
  12. Ensure compliance with all federal, state, local contracts, and programs.
  13. Serve as the Administrator in accordance with state regulations.
  14. Perform comprehensive assessments of client status, including physical, psychosocial, and environmental parameters.
  15. Develop individualized care plans based on data and scientific findings.
  16. Provide interventions focused on achieving client outcomes.
  17. Report and document changes to care plans reflecting client progress.
  18. Administer medications as needed.
  19. Maintain systematic client records.
  20. Initiate referrals to other health team members when necessary.
Minimum Requirements:
  1. Active Registered Nurse (RN) license required.
  2. Minimum one year of RN experience in NC/SC.
  3. Meet all federal, state, local contract, and program requirements, including internal certifications and training.
  4. Excellent written and verbal communication skills.
  5. At least 2 years of supervisory or management experience.
  6. Experience in quality management preferred.
  7. Proficiency with computers.
