ComForCare is seeking an Agency Supervisor/Director of Nursing Services. The DON oversees clinical compliance with NC/SC regulations and accrediting agencies, including care management and personnel management related to clinical delivery. The role also involves caregiver orientation and ongoing evaluation, ensuring clinical skills and competency across disciplines such as nursing, aides, and therapists.

Essential Duties and Responsibilities:

1. Partner with the ComForCare business owner on clinical matters
2. Engage with clients through care planning and superior customer service
3. Provide clinical oversight and case management as per regulations or business needs
4. Coordinate client care and manage transitions across the continuum
5. Oversee caregiver orientation, competency, and skills validation
6. Ensure caregiver skills match client needs regardless of setting
7. Supervise specific client, caregiver, or staff populations as directed
8. Collaborate on documentation review with the office team
9. Educate team members, patients, families, caregivers, and referral sources
10. Manage caregivers and staff during assignments, addressing concerns and providing coaching or disciplinary actions as needed
11. Participate in clinical quality initiatives
12. Ensure compliance with all relevant contracts and programs
13. Act as the Administrator in accordance with state regulations
14. Conduct comprehensive assessments of client status
15. Develop individualized care plans based on data and scientific findings
16. Provide interventions aimed at achieving client outcomes
17. Update care plans based on progress reports
18. Administer medications as necessary
19. Maintain detailed client records
20. Refer clients to other health professionals as needed

Minimum Requirements:

• Active RN license required
• At least one year of RN experience in NC/SC
• Compliance with all federal, state, local contracts, and internal certifications
• Excellent communication skills
• Minimum two years of supervisory or management experience
• Experience in quality management preferred
• Proficiency with computers
