Job summary

Home Care Network, Inc. seeks an experienced RN Case Manager for PRN visits across far-West Northwestern Ohio. You will handle starts of care, admissions, recertifications, and clinical reviews while coordinating travel to Defiance, Van Wert, Paulding and surrounding counties.

This role offers a flexible, PRN or contracted arrangement, with growth potential into part-time roles as the agency expands. Office support available in Perrysburg/Toledo as needed.

Qualifications

• Minimum 1 year of experience in case management, admissions, discharges and wound care.
• Experience with PICC/PORT/IV care and clinical assessments recommended.
• Strong documentation and field staff leadership skills required.

Responsibilities

• Manage starts of care (SOC), admissions, recertifications and transitions.
• Perform recertifications and clinical reviews across NW Ohio.
• Coordinate travel and client visits; support State Surveys as needed.
• Contribute to training, education and practice improvements.
