Job summary

Home Care Network, Inc. is seeking a Part-Time or PRN RN Case Manager/Recertification Nurse (RNCM) to coordinate home health services across Northwest Ohio, including Findlay, Fostoria, Tiffin and surrounding counties.

The role involves case management, SOC, admissions, recertifications, and supervisory duties with no overnight travel. Travel is required within a defined 30-60 minute radius or a 2-hour radius as needed.

Qualifications

• Minimum 1 year in case management, admissions, discharges, wound care, and related assessments.
• Minimum 1 year of paid experience in home health or in-home care.
• CPR and First Aid certifications required (or obtained prior to hire).
• Experience with OASIS and NON-OASIS assessments preferred.
• Active RN license valid in Ohio and ability to travel within NW Ohio.

Responsibilities

• Coordinate care and manage cases across NW Ohio with travel as needed.
• Supervise field staff and ensure timely documentation and compliance.
• Perform SOCs, recertifications, admissions, and care transitions.
• Collaborate with physicians, care teams, and payors to optimize outcomes.
