Job summary

Home Care Network, Inc. is seeking a PRN RN Case Manager to coordinate patient care, manage admissions and recertifications, and support transitions across the far-West Northwestern Ohio region. Travel to Defiance, Van Wert, Paulding, and surrounding counties is required, with occasional extended travel as needed.

The role offers a flexible, as-needed schedule and potential growth into part-time or full-time positions. RNCM experience in home health and strong documentation are essential.

Qualifications

• At least 1 year of experience in Case Management, Case Admissions and Discharges, wound care, PICC/PORT/IV care, Transitional Care, lab draws, OASIS and Non-OASIS assessments, and excellent documentation and field staff leadership.
• At least 1 year of paid experience at/with a home health agency or in-home care provider; 2–5 years preferred.

Responsibilities

• Case management, starts of care (SOC), admissions, recertifications, transitional care and ROC, clinical reviews and other assessments and services.
• Involvement in new hire orientations, in-service and supervisory reviews, and input to training and education, care practices and outcomes.
