Job summary

Home Care Network, Inc. is seeking an experienced RN Case Manager for a PRN position covering Defiance, Paulding, Van Wert and nearby counties in NW Ohio. Travel is required; schedule is flexible and mostly Mon-Fri 8-5 with no overnights. Hourly pay: $35–$40 with mileage reimbursement; scope includes RNCM and recertification visits.

This role reports to the NW Ohio Regional Administrator and offers potential progression to part‑time or full‑time in 6–12 months, plus training and QA support.

Qualifications

• At least 1 year of experience in Case Management, Admissions, Discharges, wound care, PICC/PORT/IV Care, Transitional Care, lab draws, OASIS and Non‑OASIS assessments, and excellent documentation.

Responsibilities

• Case management, starts of care (SOC), admissions, recertifications, transitional care and resumptions of care (ROC).
• New hire orientations, in‑service and supervisory reviews.
