Job summary

Home Care Network, Inc. seeks an experienced RN Case Manager to coordinate patient care in our Southeast Ohio home health program on a PRN basis. Travel to Jackson, Vinton, and Gallia Counties is required, with weekly pay and mileage reimbursement.

Qualifications

• Must have RN license in Ohio.
• Minimum 1 year of nursing supervisory experience.
• Minimum 1 year experience in case management.
• Experience in home health care, wound care, lab draws, and Oasis assessments a plus.
• Willing to travel to surrounding counties.
• Familiarity with HIPAA regulations.

Responsibilities

• Perform client assessments and coordinate care in home settings.
• Supervise a team of RN, LPN, HHA, and PCA.
• Document and communicate patient status to care team.
• Promote safety and maintain professional standards.
