Job summary

Home Care Network, Inc. seeks an RN Supervisor / RN Case Manager & Recertification Nurse (RNCM) for a regional leadership role serving Dayton and surrounding Southwest Ohio counties. The role requires travel to clients and caregivers, with supervision of case management, SOC, admissions, and recertifications.

This PRN/Part-Time position offers Mon-Fri 8am–5pm availability with occasional evenings, mileage reimbursement, and opportunities for advancement as part of a supportive team.

Qualifications

• Minimum of 1 year of nursing supervisory experience in home health or equivalent setting.
• Active RN license in Ohio; CPR/First Aid certifications.
• Experience with home health care, OASIS/NON-OASIS assessments is expected.

Responsibilities

• Coordinate patient assessments, care plans, and supervise a care team.
• Manage SOC, admissions, discharges, and recertifications.
• Travel within Dayton Metro and SW Ohio service areas; ensure regulatory compliance.
