Job summary

Home Care Network, Inc. is seeking a skilled RN Supervisor / RN Case Manager (RNCM) to serve Dayton and SW Ohio. The role is PRN or Part-Time with regional travel, reporting to the Regional Program Administrator.

Qualifications

• Must be a licensed RN in Ohio.
• Minimum 1 year supervisory/home health experience.
• Strong documentation and communication skills.

Responsibilities

• Coordinate RN case management, SOC, admissions, recertifications, and ROC; supervise nursing staff.
• Ensure timely documentation and compliance with state/federal guidelines.
• Coordinate care with physicians, care teams, and clients/families to optimize outcomes.
