Qualifications

• Current RN licensure is required.
• Experience in long-term care settings is necessary.
• Certification as RAC-CT is preferred.

Responsibilities

• Conduct comprehensive assessments of residents' health status.
• Coordinate care plans with interdisciplinary team members.
• Ensure timely completion of MDS and RAI processes.
• Train staff on MDS and RAI processes.
