Qualifications

• Current licensure as a Registered Nurse (RN).
• Experience in long-term care.
• Certification as a Resident Assessment Coordinator (RAC‑CT) preferred.

Responsibilities

• Coordinate with interdisciplinary team members to develop individualized care plans.
• Ensure accurate completion of MDS and RAI processes.
