Qualifications
- Experience in long-term care.
- Strong knowledge of MDS and RAI processes.
- Strong knowledge of Quality Measures/Quality Indicators.
Responsibilities
- Conduct comprehensive assessments of residents’ health status and care needs.
- Coordinate with interdisciplinary team members to develop individualized care plans.
- Ensure accurate and timely completion of the MDS and RAI processes.
- Collaborate with healthcare providers to implement and monitor care plans.
- Provide training and support to all staff on MDS and RAI processes.
