Qualifications

• At least 5 years health care, preferably in care coordination, and 2 years serving IRIS target groups.
• Bachelor's degree in social work, psychology, human services, counseling, nursing, special education or related field, or 5 years related experience.
• Valid driver's license, reliable transportation, and auto insurance for job travel requirements.

Responsibilities

• Lead and supervise regionally-based IRIS consultants to drive optimal outcomes.
• Demonstrate self-direction and person-centered practices.
• Use data and reports to manage IRIS services and identify proactive solutions.
• Review and authorize participant plans, budget amendments, and one-time expenses.
• Communicate DHS directives and program changes to consultants and participants.
• Maintain HIPAA confidentiality and handle sensitive information securely.
• Manage change and interpret policy or staff changes within the team.
• Collaborate with staff and stakeholders to meet service-excellence standards.
• Engage in outreach and networking with external stakeholders.
• Attend monthly leadership meetings and annual home visits with consultants.
