Job summary

CAI is seeking a Regional Grant Coordinator to support Indiana’s Rural Health Transformation Program initiatives. The role is full-time and hybrid, requiring remote field-based work within the RHTP region and occasional travel.

Responsibilities include contract management, CMS reporting, and collaboration with statewide providers. A bachelor’s degree and several years in grants/health administration are required.

Qualifications

• Bachelor’s degree in public health, health administration, public policy, business administration, or related field.
• 5 years of experience in grant management, contract oversight, program administration, or related work.
• 3 years of experience working with healthcare organizations, rural communities, or public health systems.
• 2 years of experience with compliance, budgeting, and reporting requirements for state or federal grants.
• Ability to manage multiple projects simultaneously with strong attention to detail.
• Excellent communication, facilitation, and relationship-building skills.
• Ability to work independently as a remote field-based contractor.
• Must reside within the assigned RHTP region and be willing to travel regularly within the region.
• Experience with CMS-funded programs or federal cooperative agreements.
• Familiarity with Indiana's rural health landscape, including hospitals, FQHCs, EMS, and community-based organizations.
• Experience collaborating with statewide associations or multi-stakeholder coalitions.
• Knowledge of rural health delivery, care coordination, or health system transformation.

Responsibilities

• Serve as the primary regional contract manager for organizations receiving RHTP funding.
• Monitor compliance with state and federal requirements, including expenditures and budgeting.
• Ensure regional milestones, deliverables, and timelines are met.
• Support CMS reporting requirements and audit-ready documentation.
• Coordinate with Technical Assistance Providers for aligned regional support.
• Facilitate regional meetings and cross-region learning.
• Track regional progress and assist with problem-solving to maintain momentum.
• Promote consistent use of templates and reporting tools.
