Job summary

Church World Service, Inc. is seeking an Intensive Case Manager to support refugees and vulnerable clients in navigating healthcare, housing, and community resources. The role requires hands-on case management, driving to client sites, and coordinating multi-agency services.

You will assess client needs, develop SSP-like plans, and monitor progress while maintaining strict confidentiality. A flexible schedule and reliable transportation are essential in this in-person position.

Qualifications

• Bachelor's degree in social work, public health, or related field required.
• A minimum of 3 years’ experience working with refugees or immigrants; case management preferred.
• Fluency in English mandatory; additional languages preferred.

Responsibilities

• Provide intensive case management services to special needs and vulnerable clients under ORR-funded program.
• Complete intake and enrollment assessments for clients referred to the program.
• Develop self-sufficiency plans with goals addressing health, education, and transportation.
• Coordinate referrals to local service providers and healthcare specialists.
• Advocate for client needs with community partners to improve access to services.
• Maintain detailed client case files and report program activities monthly.
