Job summary

CWS Durham is seeking an Intensive Case Manager to support refugees and vulnerable clients in Houston. The role involves assessing needs, linking to community resources, and navigating systems to promote self-sufficiency. In-person work with reliable transportation is required.

Responsibilities include intake, SSP development, referrals, advocacy, and detailed case file maintenance. Strong English skills and a willingness to engage with diverse communities are essential.

Qualifications

• Bachelor's degree in social work, public health, or related field.
• Minimum 3 years’ experience with refugees or immigrants; case management preferred.
• English proficiency required; additional languages preferred.

Responsibilities

• Provide intensive case management services to refugees and vulnerable clients in accordance with guidelines under the ORR-funded Preferred Communities program.
• Complete enrollment assessment and intake of individual clients referred to the Preferred Communities Program.
• Develop a self-sufficiency plan (SSP) with goals addressing vulnerability areas such as healthcare navigation, English learning, and transportation.
• Coordinate referrals and links to local service providers and healthcare specialists as applicable.
• Advocate for client needs, collaborating with volunteers, community partners, and coalitions to strengthen access to services.
• Provide transportation support and coordinate transportation links to promote client independence.
• Maintain detailed client case files and track/report program activities.
