Job summary

Lutheran Community Services NW seeks a dedicated Case Manager to support refugee-impacted students and families in Oregon. The role involves coordinating services with schools, maintaining case records, and helping clients access transportation and educational resources to promote school success.

The position requires fluency in English, a high school diploma or equivalent, reliable transportation, and a valid Oregon driver’s license with a clean record.

Qualifications

• Excellent English communication; read, write, speak fluently.
• Proven ability to work with diverse communities.
• Strong organizational and case management abilities.

Responsibilities

• Maintain case management for clients in accordance with contracts.
• Develop strengths-based service plans to overcome barriers and coordinate with schools.
• Provide school orientation and connect students to afterschool activities and summer programs.
• Track student grades and attendance with families and teachers/counselors.
• Provide transportation support and ensure language access for appointments.
• Coordinate referrals to refugee service providers and other agencies.
• Ensure services are linguistically and culturally appropriate and advocate as needed.
• Maintain compliance with program requirements and documentation.
