Job summary

Lutheran Community Services NW in Portland, OR is seeking a Case Manager to support refugee, asylum, and immigrant clients. You will maintain case management, develop service plans, and connect clients with community resources to foster self-sufficiency.

Responsibilities

  • Maintain case management for refugee clients in line with contracts.
  • Develop strengths-based service plans to help self-sufficiency.
  • Connect clients with community networks and supports.
  • Provide transportation for clients to appointments and ensure language access.
  • Offer in-home services including orientation, education, crisis support, and referrals.
  • Keep regular contact with active cases through home visits, meetings, and calls.
  • Maintain organized case files and timely progress notes and reports.
  • Coordinate referrals to refugee service providers and ensure cultural sensitivity.

HOW YOU WILL MAKE AN IMPACT:

All LCSNW team members will contribute to our mission of partnering with individuals, families and communities for health,justiceand hope. The Case Managerassistsand supports refugee, asylum, and immigrant clients to help them resettle and integrate into their new community and provide the resources and support to achieve self-sufficiency.

  • Maintain case management for refugee clientsin accordance withstate and federal-funded contracts
  • Work with clients to develop strengths-based service plans toidentifybarriers to self-sufficiency and outline the necessary steps to overcome these barriers
  • Connect clients with community networks and support systems to ensure their long-term integration into the community (this may include working closely with volunteers who are supporting clients)
  • Provide transportation for clients, including scheduling medical transportation or ensuring clients understand and has access to public transportation, to attend required appointments and ensure that language access is provided at these appointments
  • Provide in-home services to refugees including case management, needs assessments, cultural orientation, education, crisis support,informationand referrals as needed.
  • Keep in regular contact with active cases per contractual requirements, including home-visits, office meetings, community orientation, and phone calls
  • Maintain organized case files, completeaccurateprogress notes for all services provided,submitnecessary reportsin a timely manner, and keep shared tracking forms up to date
  • Coordinate needed services through referrals to refugee service providers including the Health Department, Employment Services, Social Security, DSHS and ESL providers
  • Ensure that clients are being served in a linguistically and culturallyappropriate mannerand provide education and advocacy when necessary
  • Maintain compliance with all relevant program/department requirements, including and not limited to adhering to all record-keeping and reporting requirements.
  • Model and encourage direct, professional communication among staff members andfacilitatedevelopment of a positive staff support network.
  • Demonstrate awareness and sensitivity in the delivery of client services as it relates to the cultural and socioeconomic characteristics of the populations served.
  • Other duties as assigned.

HOW YOU WILL BE A GREAT FIT:

  • High school diploma or equivalent isrequired.
  • Ability to work well under pressure and work effectively in a multicultural setting,demonstratingexcellent organizational skills and interpersonal skills.
  • General experiencewithGoogleSuite and Microsoft Office365.
  • This position requiresvalid state driver licenseandreliable transportation for business purposes, such as between work locations and community settings. This positionis responsible fordriving clients in a personal and/or agency rental.
  • Excellent communication skills with the ability to read, write, and speak in English.
  • Standard LCSNW criminal clearance, including national sex offender. Motor vehicle records, valid driver’slicenseand auto insurance arerequired.
