Qualifikationen

• High school diploma or GED required.
• 1–2 years of marketing or related experience preferred.
• Knowledge of home health and hospice benefits preferred.
• Familiarity with community resources and referral processes.
• Strong computer skills and ability to learn multiple systems.
• Excellent attention to detail and multitasking in a fast-paced environment.
• Strong communication, presentation, and relationship-building skills.

• Serve as the point of contact for incoming service inquiries and referrals.
• Collect, review, and document required patient information for intake.
• Coordinate nursing review of clinical information and plans of care.
• Explain available services, eligibility, and next steps to patients and families.
• Build and maintain positive relationships with referral sources and internal teams.
• Initiate and track referrals to ensure smooth start of care.
• Contribute to departmental goals and process improvements.
