Enhabit Home Health & Hospice is seeking a dedicated intake and referral coordinator to join our nationwide home-based care team. You will coordinate patient referrals, work with branch and sales teams, and ensure smooth transitions to services while applying Medicare guidelines.

The role requires a high school diploma, 2+ years in health care, and proven customer service experience. Nursing licensure is preferred; valid state driver license and reliable transportation are required.

Qualifications

• High school diploma or equivalent.
• Minimum two years in health care; one year in home health or pediatrics.
• Experience in customer service.

• Basic tech skills including operation of a mobile device.
