  • 1–2 years of marketing or related experience preferred.
  • Knowledge of home health and hospice benefits preferred.
  • Familiarity with community resources and referral processes.
  • Strong computer skills and ability to learn multiple systems.
  • Excellent attention to detail and multitasking in a fast-paced environment.
  • Strong communication, presentation, and relationship-building skills.

VNA is looking for a Referral Coordinator.

Job Summary: The Referral Coordinator plays a key role in creating a smooth and supportive experience for patients entering VNA services. This position partners closely with referral sources, discharge planners, Intake staff, patients, and families to guide referrals from initial inquiry through start of care. The Referral Coordinator ensures accurate information is gathered, clinical needs are communicated, and each referral is handled with care, efficiency, and attention to detail. This role also contributes to the ongoing growth and development of the department.

Status: Full-Time

Schedule: Monday - Friday w/ rotating Saturdays (On-Site)

Responsibilities:

  • Serve as the point of contact for incoming service inquiries and referrals
  • Collect, review, and document all required patient information to support the intake process
  • Coordinate nursing review of clinical information, including History and Physical, Plan of Care, medication reconciliation, and skilled home care needs
  • Communicate with patients and families to explain available services, eligibility, and next steps in the referral process
  • Build and maintain positive relationships with referral sources and internal teams
  • Initiate and track referrals to support a seamless transition to start of care
  • Actively contribute to departmental goals, process improvements, and growth initiatives

Education/Experience/Skills/Qualifications:

  • High school diploma or GED required
  • 1-2 years of marketing or related experience preferred
  • Knowledge of home health and hospice benefits preferred
  • Familiarity with community resources and referral processes
  • Strong computer skills with the ability to learn multiple systems
  • Excellent attention to detail with the ability to multitask in a fast-paced environment

The compensation range for this role is determined by factors such as location and required skills. The salary range provided reflects the lowest to highest amount we reasonably anticipate paying for this position at the time of posting. Compensation is influenced by labor market conditions and various factors, including but not limited to: prevailing minimum wage in the area, relevant education, experience, skills, geographic location, performance, travel requirements, sales or revenue targets, and business needs. It is not typical for a new hire to be offered the highest end of the salary range. Pay decisions are based on the unique circumstances of each candidate and position.
