The Referral Coordinator plays a key role in creating a smooth and supportive experience for patients entering VNA services. This position partners closely with referral sources, discharge planners, Intake staff, patients, and families to guide referrals from initial inquiry through start of care. The Referral Coordinator ensures accurate information is gathered, clinical needs are communicated, and each referral is handled with care, efficiency, and attention to detail. This role also contributes to the ongoing growth and development of the department.
