Job summary

Hartford HealthCare Medical Group is seeking a Referral Coordinator to manage new patient referrals to the Primary Care Behavioral Health service line. You will schedule based on location preferences, review referrals for completeness, and support front desk staff, providers, and managers across multiple offices.

Responsibilities include maintaining patient records with confidentiality, obtaining prior authorizations, and communicating with patients, doctors, and insurers to keep referrals

Qualifications

• High School Diploma or higher preferred.
• Minimum of 1 year as a medical receptionist or previous care coordination experience.
• Direct customer service in the medical field preferred.

Responsibilities

• Registers and schedules patients.
• Collaborates and communicates with PCBH teams to ensure continuity of medical care, including referral requests and scheduling.
• Assists with creating standard work for referral coordination.
• Responds to emails, website inquiries, and calls to coordinate patient appointments.
