Job summary

Tampa Family Health Centers is seeking a Referral Specialist to support the coordination, scheduling, and follow-up of patient referrals generated by primary care providers. This role ensures timely processing of referrals, maintains accurate documentation, and partners with clinical and administrative teams to support continuity of care and patient access to specialty services.

Key duties include tracking referrals, verifying eligibility, and communicating status updates to care teams.

Qualifications

• High school diploma or equivalent required.
• Minimum 1 year of referral, patient access, or healthcare administrative experience.
• Previous medical front desk or insurance verification experience preferred.

Responsibilities

• Process and schedule patient referrals in a timely manner based on urgency.
• Track and follow up on referrals to ensure completion and compliance.
• Maintain accurate referral logs and documentation in the EHR.
• Collaborate with providers, case managers, and external partners on referral coordination.
• Verify patient eligibility and assist with referral-related documentation.
• Communicate referral status updates to providers and care teams.
• Support patients with referral education and next steps.
• Assist with urgent referrals and department calls as needed.
