Tampa Family Health Centers is seeking a Referral Specialist to support the coordination, scheduling, and follow-up of patient referrals generated by primary care providers.

This on-site with hybrid role ensures timely processing and accurate documentation to help patients access specialty services. You will collaborate with clinicians and administrative teams, verify eligibility, and maintain logs in the EHR.

Qualifications

• High school diploma or equivalent required
• Minimum 1 year of referral, patient access, or healthcare administrative experience
• Previous medical front desk or insurance verification experience preferred

Responsibilities

• Process and schedule patient referrals in a timely manner based on urgency
• Track and follow up on referrals to ensure completion and compliance
• Maintain accurate referral logs and documentation in the EHR
• Collaborate with providers, case managers, and external partners on referral coordination
• Verify patient eligibility and assist with referral-related documentation
• Communicate referral status updates to providers and care teams
• Support patients with referral education and next steps
• Assist with urgent referrals and department calls as needed
