Job summary

Swope Health Services is seeking a Referral Coordinator to manage referrals to outside services and coordinate patient appointments. You will verify information, collect insurance data and authorizations, and ensure timely communication with providers, specialists, and patients.

The role requires strong communication skills, previous medical office experience, and BLS certification where possible. Join a team focused on coordinated, patient-centered care and documentation in the EMR system.

Qualifications

• High school diploma or equivalent required.
• Experience in a medical office preferred.
• BLS certification preferred.
• Excellent communication and telephone etiquette.

Responsibilities

• Process and track referrals to outside PCMH department.
• Enter referral information including insurance and authorization codes.
• Coordinate calls with providers/specialists and ensure appointment notification.
• Contact patients with referral details and reschedule instructions.
• Document all interactions in the Notes section and EHR (eCW).
• Track referrals until they are marked Pending per policy.
