Qualifications

• Two years’ healthcare experience, preferably in referral coordination or related admin role.
• Proficiency with EMR systems (Epic, Cerner) and Microsoft Office.
• Knowledge of HIPAA and healthcare privacy regulations.
• Excellent verbal and written communication, professionalism with patients and providers.

Responsibilities

• Process, coordinate, and follow up on patient referrals to ensure timely transitions of care.
• Identify and address delays in referral workflows; implement improvements.
• Schedule appointments with specialists and external providers based on patient needs.
• Ensure all necessary medical records and documents are provided timely.
• Verify insurance coverage and obtain pre-authorizations as needed.
• Maintain relationships with external providers to facilitate referrals.
• Communicate referral status and appointment details to patients.
• Document all referral-related actions in the EMR system.
• Maintain organized records and report referral statuses as required.
• Ensure HIPAA compliance in handling patient information.
