Job summary

JEVS Human Services Career Center is seeking a Resource Coordinator to support participants in the reentry program, develop Individual Anti-Violence plans (IAP), and provide case management guidance through the VIP program.

The RC will work with referrals from state and county agencies, facilitate VIP focus groups, coordinate wrap-around supports, and help clients achieve employment and stability.

Responsibilities

• Assists SAS clients with identifying potential barriers to employment.
• Develop clients’ Individual Anti-Violence plans (IAP).
• Facilitate daily VIP focus groups as part of the JEVS SAS VIP Program.
• Assist clients in reaching their goals in the program by providing retention services, stressing good attendance, punctuality, and promotion of a positive work ethic.
• Provides regular, ongoing individual counseling to address barriers, encourages achievement of goals and ensures retention of clients.
• Maintains DAP notes regarding clients’ progress and prepares statistical reports as required by the program.
• Follows procedures and policies as specified by program regulations.
• Meets and establishes rapport with support agency partners and referred customers within established timeframes.
• Assesses customer needs and provides appropriate support, referrals, and participant follow up.
• Facilitates coordination of services throughout the continuum of care.
• Network and establish working relationships with appropriate personnel at the Philadelphia, Montco County Assistance Offices, Community Agencies, Mental Health and Drug and Alcohol Organizations and other agencies as needed.
• Act as a referral agent, referring clients to external agencies and organizations for assistance with behavioral health, issues, housing, employment, legal or family related issues. And provide follow-up referral assistance.
• Guide participants in learning how to independently resolve and avoid issues, problems involving violence.
• Be on-call after works hours to intervene at the time of a violent occurrence if needed.
