Job summary

Breakthrough is seeking a Case Manager, Violence Prevention in Chicago, IL to support a caseload of participants including the reentry population.

Working with the Outreach Team, you will provide client-centered case management, employment and education assistance, life skills development, crisis intervention, and referrals to community resources in individual, family, and group settings to help participants reintegrate and thrive.

Qualifications

• Bachelor's degree preferred in a Human Services field or equivalent experience.
• Experience working with at-risk youth and street involved youth.
• Knowledge of trauma reactions and restorative justice concepts.

Responsibilities

• Identifies and develops relationships with at risk individuals within target community
• Engages participants in interviews to assess needs and determine referrals
• Coordinates internal and external services for participants
• Monitors cases and advocates for needed services
• Supports job readiness workshops including resume and interview skills
• Maintains participant records in an online data system
• Communicates progress in weekly interdisciplinary meetings
• Prepares exit strategies and coordinates post-discharge resources
• Develops partnerships with community resources
• Adheres to confidentiality and safety protocols
