Job summary

RHA Health Services, LLC seeks a Community Reentry Case Manager to support clients with behavioral health needs transitioning to community living. You will coordinate with the Buncombe County Detention Facility and community providers to assess, plan, and implement strategies reducing recidivism and promoting safety.

You will conduct empathetic interviews, manage data reporting, and collaborate with partners to ensure continuity of care.

Qualifications

• Qualified Professional status with relevant credentialing.
• Experience with the Criminal Justice system and justice-involved populations is preferred.
• Strong communication, organizational, and data-reporting skills are required.
• Ability to navigate complex systems and engage individuals in services with sensitivity.

Responsibilities

• Direct Client Care and support to individuals with behavioral health needs.
• Coordinate planning and linkage to services pre- and post-release.
• Access community resources and coordinate with providers.
• Provide case management and liaison roles to maintain service coordination.
