Job summary

Horizon Behavioral Health in Lynchburg, Virginia, is seeking a Case Manager for the Reentry Grant to support justice-involved adults with substance use disorders as they reenter the community. You will conduct assessments, develop individualized reentry plans, and coordinate evidence-based treatment and recovery supports to ensure continuity of care from incarceration to community reintegration.

Qualifications

Bachelor's degree in an applicable human services field.
Minimum of one year of relevant mental health, developmental disability, and/or substance use work experience with the identified population.

Responsibilities

Manage a caseload of justice-involved adults with substance use disorders transitioning back to the community.
Conduct comprehensive needs assessments and develop reentry service plans.
Coordinate evidence-based treatment and recovery supports.
Ensure continuity of care from incarceration through community reintegration.
Collaborate with correction staff, healthcare providers, peer recovery specialists, probation and parole, housing agencies, employers, and other community partners.
