Job summary

CHJ Inc. in Illinois seeks a dedicated Case Manager to support the Community Reentry (CR) program. You will deliver addiction intervention, coordinate services, and recovery-supportive strategies for clients and families.

Qualifications

• High school diploma or GED required.
• CADC certification within two years of employment may be required.
• Must pass an IDOC background check.
• Proficiency with Microsoft Office.
• Experience in group facilitation.
• Knowledge of ASAM criteria and DSM-V related to substance use disorders, or willingness to learn.
• Comfort working in a correctional setting and with individuals convicted of serious offenses.
• Strong time management and communication skills.
• Ability to collaborate with internal and external partners.

Responsibilities

• Assist individuals in achieving service plan goals through direct services and coordination with behavioral health, public agencies, and community resources.
• Assess progress and adjust service plans in collaboration with parole and other authorities.
• Complete intake packets and program enrollment.
• Provide referrals and crisis intervention for identified needs.
• Interview individuals and review progress with professionals to determine suitability for participation.
• Collaborate with internal and external partners and participate in staff meetings and integrated staffing.
• Assist clients with enrollment in public benefits and obtaining vital records.
• Maintain all required documentation including progress notes and discharge summaries.
• Ensure confidentiality of information and program compliance.
• Monitor progress toward service goals and preparedness for transition.
