Community Connections, Inc. is seeking a dedicated Case Manager for the ReEntry program in the District of Columbia. The role delivers intensive, community-based behavioral health case management to adults transitioning from incarceration to community living, using trauma-informed, recovery-oriented approaches.

Hybrid role with a pay range of $48,000–$59,000, depending on experience. You will manage 20–25 participants, coordinate care across behavioral health, housing, and employment services,

Formación

• Bachelor's degree in Social Work, Psychology, Counseling, Sociology, Criminal Justice, or related field.
• Minimum 2 years of experience in behavioral health, case management, or social services to adults.
• Experience with serious mental illness and co-occurring substance use disorders.
• Experience working with justice-involved populations.
• Ability to work independently in community settings.
• Proficiency with electronic documentation systems.

Responsabilidades

• Maintain an active caseload of approximately 20-25 participants and provide intensive support during pre-release planning and the first six months following community reentry.
• Coordinate behavioral health, substance use treatment, primary care, psychiatric, and social service appointments.
• Develop and implement individualized transition and recovery plans.
• Facilitate linkages to Medicaid, housing, employment and benefits, and other community resources.
• Engage participants, monitor stability, and document progress in electronic health records.
• Provide crisis support and ensure access to essential services during high-risk transitions.
