Job summary

HealthRIGHT 360 is seeking a Community Service Representative II to support CDCR clients with case management, referrals, and reentry services. You will screen, assess, and connect individuals to community resources, and help them progress toward independent, crime-free living.

You will maintain CMPs, coordinate placements, ensure accurate records, and collaborate with a multidisciplinary team while traveling to community-based sites as needed.

Qualifications

• Experience working with CDCR or similar criminal justice population.
• SUD treatment/recovery program experience.
• First Aid and CPR certification within 30 days of employment.
• Ability to maintain data integrity and confidentiality.

Responsibilities

• Maintains an average caseload of CDCR clients; develops CMPs and tracks progress.
• Assesses and coordinates placements into community providers; coordinates transportation.
• Files and documents case notes in accordance with policies and external requirements.
• Travels to CBPs and conducts outreach; assists in training new hires.
