Job summary

Pioneer Human Services is seeking a Residential Reentry Center Case Manager to plan and manage reentry/transition plans for residents referred from FBOP, USPO, and USPT. The role emphasizes employment, housing, and treatment access to support successful community reintegration.

The position uses evidence-based, client-centered practices including LS/CMI assessments and SMART goals, with fieldwork, on-site duties, and collaboration with staff and external agencies.

Qualifications

• BA/BS degree in human services or related field required.
• 2 years of experience in human services or related field.
• Must become certified in LS/CMI risk-needs assessment within 90 days of hire.
• Must complete PREA training within first month of hire.
• Must become certified in Moral Recognition Therapy within one year.
• Must pass and maintain security clearances as required by FBOP.
• Must pass CPR/First Aid Certification.
• Must have a valid driver’s license and carry auto liability insurance.

Responsibilities

• Perform comprehensive LS/CMI risk-needs assessment and PREA Risk Assessment.
• Orient residents to program rules and expectations.
• Develop IPPs using SMART goal techniques.
• Align IPP interventions to address criminogenic needs.
• Provide regular case notes documenting performance.
• Make referrals for mental health, housing, treatment as needed.
• Conduct field work including home investigations and checks.
