Job summary

The People Concern is seeking a Case Manager for the Jail Program (ODR) in Sherman Way, Van Nuys, CA. You will provide intensive case management to clients recently released from jail who are homeless, coordinating release logistics and helping secure permanent supportive housing in the community.

You will work intensively with a diverse client group, ensuring trauma-informed care and wrap-around services while maintaining comprehensive client files and adhering to HIPAA guidelines.

Qualifications

• Bachelor's degree preferred; or a minimum two years' case management experience working with individuals experiencing homelessness
• Demonstrated knowledge of case management
• Able to work in a high tolerance Harm Reduction model with clients who have multiple barriers
• Skill in non-violent crisis intervention
• Computer literate with basic Word, Outlook, and Excel skills
• Detail oriented with strong time management and organizational skills
• Able to obtain CPR/First Aid certification

Responsibilities

• Work with a diverse and marginalized client caseload of 15 justice-involved clients experiencing many barriers
• Maintain weekly contact with all clients and document contact in County approved database
• Link clients to medical, mental health, substance use, and vocational resources
• Assist clients in obtaining benefits to which they are entitled
• Coordinate appointments, transportation, and follow-up services for clients
• Develop effective relationships with clients to improve housing stability and well-being
