Job summary

The People Concern in Los Angeles is seeking a Case Manager to work with clients recently released from jail, helping them move from homelessness to permanent supportive housing through intensive case management.

In partnership with DHS/ODR, the role focuses on trauma-informed care, harm reduction, and coordinating health, housing, and community services to support independence and housing stability. 100% onsite, Mon–Fri, 8:00am–4:30pm, located at Vanowen Street, North Hollywood.

Qualifications

• Bachelor's degree preferred or a minimum two (2) years' case management experience working with individuals experiencing homelessness.
• Experience working with individuals with mental illness and/or substance use issues is a plus.
• Strong knowledge of case management practices and documentation in databases.

Responsibilities

• Work with a diverse caseload of clients transitioning from homelessness to permanent housing.
• Maintain weekly contact with clients and document in the County approved database.
• Provide trauma-informed, harm-reduction focused case management.
• Link clients to medical, mental health, substance use, and vocational resources.
• Assist clients in obtaining benefits to which they are entitled.
• Maintain comprehensive client files and update biopsychosocial assessments.
