Job summary

The People Concern is hiring a Case Manager for the ODR Project Based Housing program in Los Angeles. This full-time onsite role supports clients recently released from jail as they transition to permanent housing.

You will conduct intensive case management, coordinate resources, and document care within HIPAA-compliant systems. Working 8:00am-4:30pm, Monday–Friday, you’ll collaborate with DHS and ODR teams, maintain detailed records, and advocate for clients’ housing stability and wellbeing in

Qualifications

• High School Diploma or GED or equivalent.
• Bachelor’s degree preferred; or a minimum two (2) years’ case management experience.
• Valid CA Driver’s license, reliable car, auto insurance, and an acceptable driving record.
• Able to transport clients in your vehicle.
• Able to pass a background check for jail clearance.
• Demonstrated knowledge of case management.
• Able to work in a high tolerance Harm Reduction model with clients who have multiple barriers.
• CPR/First Aid certification.

Responsibilities

• Work with a diverse and marginalized client caseload of 15 justice-involved clients with multiple barriers.
• Maintain weekly contact with all clients and document contact in County approved database.
• Provide services reflecting the “Whatever It Takes” model emphasizing choice, harm reduction, trauma-informed care.
• Link clients to medical, mental health, substance abuse, and vocational resources.
• Assist clients in obtaining all benefits to which they are entitled.
• Participate in court hearings and submit progress reports.
• Develop and maintain up-to-date biopsychosocial assessments.
• Create goal plans and review progress quarterly and on completion.
• Maintain complete, current client files in agency records and databases.
• Document all client contacts in PIP format.
