Job summary

The People Concern is seeking a Case Manager for the ODR Project Based Housing in Los Angeles. This role involves working with justice-involved clients transitioning from homelessness to permanent supportive housing, providing intensive case management and support for diverse needs.

Responsibilities include maintaining regular client contact, developing comprehensive care plans, and linking clients with essential community resources. A Bachelor's degree is preferred, along with experience in case management and transportation capabilities.

Qualifications

• Bachelor's degree preferred or minimum 2 years' case management experience.
• Valid CA Driver's license and reliable vehicle required.
• Knowledge of case management practices is essential.

Responsibilities

• Work with a caseload of 15 justice-involved clients.
• Maintain weekly contact and document in databases.
• Link clients to appropriate health and social resources.
