The People Concern in Long Beach is seeking a Case Manager for the ODR Project Based Housing program. The role supports clients recently released from jail as they move from homelessness to permanent supportive housing, offering intensive case management and wrap-around services.

Responsibilities include coordinating care with DHS/ODR teams, maintaining documentation, and helping clients access health, mental health, and social services.

Qualifications

• Bachelor's degree preferred or equivalent case management experience.
• Valid CA driver's license and reliable vehicle.
• Ability to transport clients in your vehicle.
• Experience working with homelessness and trauma-informed care is a plus.
• Proficient computer skills and database usage.

Responsibilities

• Maintain a caseload of up to 20 clients transitioning to permanent housing.
• Maintain weekly client contact and document in the database.
• Provide services reflecting a harm-reduction, trauma-informed approach.
• Link clients to medical, mental health, substance abuse, and vocational resources.
• Assist clients in obtaining benefits and managing appointments.
• Attend court hearings and submit progress reports.
• Develop biopsychosocial assessments and update quarterly.
