Job summary

Amity Foundation in Downtown Los Angeles seeks a compassionate Intensive Case Manager to help build a network of housing, mental health, and reentry services, aiming to prevent reincarceration and support long-term housing goals. This on-site, full-time hourly role will serve clients referred by the diversion and reentry offices.

Qualifications

• Bachelor’s degree or equivalent experience required.
• Experience in outreach, counseling, reentry services & case management.
• Ability to work with culturally diverse populations.

Responsibilities

• Interview and assess participant needs referred by diversion and reentry.
• Conduct wellness checks and provide crisis management.
• Develop treatment plans to address service needs and goals.
• Advocate for public benefits and linkage to resources.
• Coordinate housing, mental health, and substance abuse services.
• Collaborate with housing staff, probation officers, psychiatrists, and public defenders.
• Attend inter-agency meetings to support community cooperation.
• Maintain concise case records and timely documentation.
• Use company or personal vehicle to visit county jails and transport clients.
