Job summary

The People Concern in Long Beach, CA is seeking a Case Manager for the ODR Project Based Housing team. The role supports clients recently released from jail, moving from homelessness to permanent supportive housing, delivering intensive case management and advocacy.

You will work with a diverse caseload, collaborate with health and social services, coordinate transportation, and help clients access benefits, housing stability, and independent living skills in a trauma-informed, harm-reduction

Qualifications

  • Bachelor's degree preferred; or a minimum two (2) years' case management experience with homelessness, preferably with mental illness/substance use, or an equivalent combination of education and experience
  • Valid CA Driver's license, reliable car, auto insurance, and an acceptable driving record
  • Able to transport clients in your vehicle
  • Able to pass a background check for jail clearance
  • Demonstrated knowledge of case management
  • Able to work in a high tolerance Harm Reduction model with clients who have multiple barriers
  • Skill in non-violent crisis intervention
  • Computer literate; able to effectively use computerized database for client file management, with basic skills in Word, Outlook, and Excel
  • Detail oriented with strong time management, organizational, written, verbal, interpersonal, and computer skills
  • Able to obtain and maintain CPR/First Aid certification
  • Minimum 30 WPM typing speed
  • Bachelor's degree

Responsibilities

  • Work with a diverse and marginalized client caseload of 20 justice-involved clients experiencing many barriers including homelessness, trauma, medical, mental health and substance use.
  • Maintain weekly contact with all clients and document contact in County approved database.
  • Ensure that case management services reflect the Whatever It Takes model emphasizing customer choice, harm reduction, trauma-informed care and psycho-social rehabilitation.
  • Link clients to medical, mental health, substance abuse, and vocational resources.
  • Assist clients in obtaining benefits to which they are entitled.
  • Participate in court hearings and submit court progress reports.
  • Ensure each client on caseload has an up-to-date biopsychosocial assessment.
  • Develop and review goal plans to improve housing stability and overall well-being.
  • Maintain a complete, accurate, current client file with documents and data in agency records and electronic databases.
  • Document all client contacts in PIP format and per Housing Department productivity expectations.
  • Maintain confidentiality of client files per HIPAA and applicable guidelines.
  • Coordinate appointments, transportation, and follow-up services for health care and community resources.
  • Transport clients to agencies and providers in the greater Los Angeles area to increase community support and reintegration.
  • Develop trusting relationships focusing on independence and health maintenance.
  • Maintain knowledge of community resources and utilize them for wrap-around services.
  • Assist clients with daily activities, scheduling, budgeting, socialization, health maintenance, and recovery progress.
  • Collaborate with staff and participate in weekly meetings and trainings.
