Job summary

The People Concern is seeking a Case Manager – Jail Program (ODR) to provide intensive case management for clients recently released from jail, helping them obtain permanent supportive housing in the community. The role partners with DHS/ODR and operates in the Sherman Way Van Nuys location.

The ideal candidate coordinates medical, behavioral health, and social services, maintains thorough documentation, and supports clients toward stability in housing and health outcomes.

Qualifications

• Bachelor’s degree preferred or equivalent experience.
• Minimum two years of case management experience with homelessness or reentry clients.
• Valid CA Driver’s license and reliable vehicle required.

Responsibilities

• Provide intensive case management to justice-involved clients experiencing homelessness.
• Maintain weekly contact and document activities in the agency database.
• Coordinate medical, mental health, and substance use services and housing resources.
• Assist with court processes and ensure up-to-date biopsychosocial assessments.
• Develop individualized goal plans and monitor progress toward housing stability.
