Job summary

HealthRIGHT 360, a nonprofit focused on integrated health services for vulnerable populations, seeks a Case Manager for the STOP Area Six program in California. You will support CDCR clients, coordinate placements, and maintain accurate documentation with a multidisciplinary team to promote health and recovery.

Responsibilities include caseload management, CMP development, community referrals, and liaising with parole staff.

Qualifications

• Two years of case management experience with criminal justice populations.
• Bachelor’s degree in Social Work, Psychology or related field.
• Substance Use Disorder Counselor registration/certification (CCAPP/CAADE/CADTP).
• First Aid and CPR certification or ability to obtain within 30 days of hire.
• Valid California Driver’s License and access to transportation preferred.

Responsibilities

• Maintain an assigned caseload of up to 50 CDCR clients or more as needed.
• Develop Case Management Plans (CMP) and monitor progress through program phases.
• Coordinate placements into appropriate community resources and CBPs.
• Communicate with Parole Agents and document client progress in files.
• Provide training support and assist with new hires when needed.
• Obtain and review necessary paperwork to ensure treatment access.
